In general, appointments may be divided into consultation, surgical and follow up. Your initial appointment may consist of a consultation explaining your diagnosis and probably treatment. Under certain circumstances, surgery can be done on the same day as the consultation; however, this requires advance planning and communication between our office staff.

After making your appointment, if you have the capabilities to e-mail the Patient and Insurance Information to us, please do so immediately. Also, please inform the receptionist to expect the information. On your appointment date please arrive at least 15 minutes early and bring with you:
- X-Rays if applicable
- A list of medications you are presently taking
- Medical and Dental Insurance Cards.
If you do not have internet e-mail access, you will receive via mail a packet of information that you need to complete and bring with you on your appointment date. Completion of all this information will speed up your registration process. Please arrive 15 minutes earlier than your appointment time. If you find it necessary to cancel your appointment, we would appreciate the courtesy of 24-hour notice.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A pre-operative consultation and physical examination is mandatory for patients undergoing IV general anesthesia for surgery. Please have nothing to eat or drink 6 hours prior to your surgery. You will also need an adult to stay in the office during your surgery and to drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) |
